How do I print completed online enrollment forms?

Description

We like to keep a copy of the completed online enrollment form on in the child’s record. How can I print the form that the parent completed?

Solution

The easiest way to print a copy of the completed online enrollment form is to print it from the Review/Approve section in CORE.

1. Log into CORE: https://elv.earlylearningventures.org/elv/login
2. Click on Provider Management (purple navigation pane)
3. Click on Online Enrollment tab at the top
4. Click on Online Enrollment Review/Approve
5. Find the name of the family you want to print
6. Click once on the name
7. Click Report at the bottom of the page (yellow button) – The report will download as a PDF
